Employees in Poland who regularly use a display screen monitor or laptop as part of their work may be eligible for reimbursement of the cost of corrective glasses.
Eligibility, reimbursement limits, and the required documentation are subject to the conditions outlined in the company's corrective glasses reimbursement policy.
Who is eligible?
You may be eligible for reimbursement if both of the following conditions apply:
You use a display screen monitor or laptop for at least half of your working time, and the display screen is a permanent part of your workstation.
An ophthalmological examination completed as part of your preventive occupational health care, or an examination by a specialist doctor, confirms that you need corrective glasses while working with a display screen monitor or laptop.
How much can be reimbursed?
The reimbursement covers the actual cost of the corrective glasses, up to a maximum of 400 PLN.
The maximum reimbursement amount is 400 PLN.
Reimbursement is available once per year.
Costs above the 400 PLN limit are not reimbursed.
Are contact lenses covered?
Corrective contact lenses may also be covered where, following a referral by the employer, a doctor determines that the employee's vision defect cannot be corrected with glasses.
What documentation is required?
To request reimbursement, you must provide:
A specialist doctor's certificate confirming that you need corrective glasses while working with a display screen monitor or laptop.
An invoice or receipt for the purchase of the glasses.
A completed Corrective Glasses Reimbursement Application.
The invoice or receipt must be issued to:
HORIZONS GLOBAL TECHNOLOGY Sp. z o.o.
The reimbursement application requires your employee details, supporting documentation, the reimbursement amount requested, and your bank account details.
Who needs to approve the application?
The reimbursement application must be reviewed and approved as part of the employer's process.
Your company manager or administrator must confirm that you:
Perform your official duties using a display screen monitor or laptop for the required amount of working time.
Meet the applicable conditions for reimbursement.
Please ensure that the required confirmation or signature is obtained before submitting your request.
When should I submit my application?
Your application must be submitted within 30 days from the date your specialist doctor's certificate is issued.
We recommend obtaining and retaining your medical certificate before purchasing your glasses and submitting your reimbursement request as soon as possible once you have all the required documentation.
How do I submit my reimbursement request?
Once you have purchased your glasses and collected all the required documentation:
Complete the Corrective Glasses Reimbursement Application (attached below)
Attach your specialist doctor's certificate.
Attach the invoice or receipt for the glasses, issued to HORIZONS GLOBAL TECHNOLOGY Sp. z o.o.
Obtain the required manager or client representative approval.
Submit the completed application and supporting documents through the Expenses section of the Remote People platform.
Your request will be reviewed and approved before being processed for reimbursement.
